Waiver and Release
I agree to grant Studio T Arts & Entertainment permission to capture and use photos and videos of me. *
I hereby release {STAE Entity}, its respective administrators, directors, agents, officers, volunteers, employees, other participants, any sponsors, and, if applicable, owners and lessors of premises on which the Activity takes place, (each considered one of the "RELEASEES" herein) from all liability, for injury, death, and property loss and damage that arises out of or results from the activity or event, including all liability which results from the NEGLIGENCE of PROVIDERS, or any other person or cause.
I have read the RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT and understand that I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law and agree that if any portion of this agreement in held to be invalid the balance, notwithstanding, shall continue in full force and effect.
MODEL RELEASE
I hereby give permission to {STAE Entity} to use my name and photographic likeness in all forms and media for advertising, trade, and any other lawful purposes.
PARENTAL CONSENT
AND I, the minor's parent and/or legal guardian, understand the nature of the above referenced activities and the minor's experience and capabilities and believe the minor to be qualified to participate in such activity. I hereby release, on behalf of the minor child, {STAE Entity}, its respective administrators, directors, agents, officers, volunteers, employees, other participants, any sponsors, and, if applicable, owners and lessors of premises on which the Activity takes place, (each considered one of the "RELEASEES" herein) from all liability, for injury, death, and property loss and damage that arises out of or results from the minor child’s participation in the activity or event, including all liability which results from the NEGLIGENCE of PROVIDERS, or any other person or cause. I release photos and/or video of Release taken before, after, or during activity to be used in program promotion and marketing only.
All digital content created in the STAE LVL UP, Side Hustle and PassToClass training programs are the sole property of Studio T Arts & Entertainment and cannot be used without depressed, written permission from Studio T Arts & Entertainment.
